About this data: how every number here is verified
What counts as a source
Every value on this site is quoted verbatim from a primary source: the payer's own administrative guide or published policy page, the California regulation or statute, or the federal regulation. We do not cite billing blogs, vendor round-ups, or paraphrases. Each table row shows the exact sentence the value comes from, a link to the document, and the document's stated effective date.
Verification method
Before publication, every source document is re-retrieved and the quoted sentence is checked against the live text — inherited citations are not trusted. Where a payer's published number conflicts with a governing regulation (for example, a national appeal window that is shorter than a California dispute-filing floor), both are recorded and the conflict is noted, because the regulator that governs the specific product decides which clock controls.
What we do when a value cannot be verified
If a rule is reported only by secondary sources, we do not publish the number. The row appears with the instruction to confirm the rule in your provider manual instead. A value that cannot be traced to a primary source is treated as unknown, not as approximately right.
Day types are load-bearing
Calendar days, business days, and working days are distinct counting systems, and month-based windows are never converted to day counts — a 12-month window is not the same thing as a fixed day count. Every value on this site keeps the unit its source uses.
Corrections
If you find an error — a superseded rule, a changed manual, a wrong citation — we want to know. Corrections are logged with the correcting source, and every affected page is regenerated from the corrected record. Reach us through the link in the footer of any page.
Who maintains this reference
Appealant's billing and revenue-cycle team builds and maintains this reference from payers' own published provider documents and the governing state and federal regulations. Every value is re-retrieved from its primary source before publication, and superseded rules are removed as sources change.
